This form can be submitted electronically (e.g. via a secure contact form using your user account with login via the electronic ID function or the ELSTER certificate) or handwritten and signed in paper form to the responsible authority.
You can apply for a grant for your fertility treatment (in vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) treatment).
Funding is available for the costs of in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) treatments for the first to fourth treatment cycle. The aim is to make it easier for couples with an unfulfilled desire to have children to access reproductive medicine.
Couples receive a subsidy towards the costs of fertility treatment, depending on their remaining treatment costs after settlement with their health insurance provider.
Beneficiaries are heterosexual married couples or heterosexual couples living in a non-marital partnership. Both partners must have reached the age of 25. The woman must be under the age of 40 and the man under the age of 50.
The couple must have their joint main residence in Bavaria.
Couples who are not married must be in a long-term partnership. A long-term cohabitation does not permit any further cohabitation and is characterized by an inner bond. It can be assumed if - according to the declaration of the unmarried couple - they live together in a stable partnership and the unmarried man declares that he will recognize the paternity of the child conceived through reproductive medicine.
The allowance for married couples is up to 50 percent of the remaining personal contribution for all four treatment cycles, for couples in a non-marital partnership up to 25 percent for the first to third treatment cycle and up to 50 percent of the remaining personal contribution for the fourth treatment cycle.
The maximum amounts for all couples are EUR 800 for IVF treatment in the first to third treatment cycle and EUR 1,600 in the fourth treatment cycle, and EUR 900 for ICSI treatment in the first to third treatment cycle and EUR 1,800 in the fourth treatment cycle.

It takes around three to four weeks from receipt of your funding application to receipt of the grant notification. If additional documents need to be requested, it may take longer. Please take this into account when planning the timing of your fertility treatment.
Please note that the measure is only eligible for funding if the treatment of the respective eligible treatment cycle, including drug treatment, has not yet begun. The purchase of medication or the filling of prescriptions required for fertility treatment counts as the start of the measure. This may only take place after receipt of the grant notification.
The preparation of the treatment plan and the declaration of assumption of costs by the statutory health insurance fund, the subsidy or the private health insurance fund are not considered to be the start of treatment for the purposes of these guidelines.
A separate grant must be applied for each treatment cycle and, if applicable, each repeat attempt.
After the birth of a child, the treatment cycles are again eligible for funding if the pregnancy leading to the birth of the child occurred within the four eligible treatment cycles.
